Your 5 Year CME Submission

This upload page allows you to submit proof of your 30 hours of completed Lifestyle Medicine-Specific CME.

Diplomates who have failed to submit the 30h of CME each 5 year period will have until March 31st in year 6 to rectify the situation. Failing to do so will revert to the original recertification pathway.

• For drop-outs within the first 5 years, the original 10 year anniversary applies.
• For drop-outs after the first 5 years, recertification is due 5 years from the drop-out date.
• There is no option to “buy back in”, i.e. drop-outs are no longer eligible for the MOC pathway.
